Primary Prevention Integration Plan – ready to take the next step?August 14, 2012 0 comments
After our RPE Conference last week, it’s time to move to the next phase of the process: creating an action plan. Following the conference, California RPE grantees will be filling out this Primary Prevention Integration Action Plan to submit to the California Department of Public Health (CDPH) along with their year end report. CDPH will then ask that each grantee select at least one objective from their plan that they could do to improve upon integration of primary prevention throughout their agency to include in their CDPH Scope of Work for the upcoming grant cycle (2012/2013).
August – October: Meet with staff at your agency to figure out which pieces of integration are most important for your agency. Maybe it’s restructuring staff meetings? Creating a facebook page and outlining which staff members going to manage and create content for social media tools? Training the Board of Directors?
August: Write your CDPH application Scope of Work for 2012/2013. CDPH has inserted a placeholder objective that is VERY broad. Why so broad? You won’t have completed your Primary Prevention Integration Action Planby the time you need to submit your application. The broad objective will serve as a placeholder so that whenyou have finished your Integration Plan, you’ll be able to work on the objective(s) that YOU’VE identified as being the most important and of highest priority for your agency.
October: Fill out and submit the Primary Prevention Integration Action Plan.
November: When you get up and running with your new CDPH grant cycle, you can get moving with the objective(s) that you’ve identified in your Primary Prevention Integration Action Plan.
